In 2022, the Stoughton Police Department began a program to register at-risk individuals. As our program continues to grow, we want to reach not only at-risk adults but also children.
Below you can find a new updated form for the program.
This information will be retained and distributed to first responders for the sole purpose of identifying and assisting the person at risk should they wander, need assistance, or experience any other type of emergency.
